Children and Youth in Transition; A Toolkit for Effective Transitions, is intended to assist with and improve social worker and foster parent practices when supporting children-in-care during times of residential transition. The Transition Subcommittee hopes that the information presented here will be used as a guide for planning and implementing transitions and will be shared with others.
